NY Care Consultants understands that the health care system can be a fragmented and difficult place to navigate. Care transitions, whether it be home to hospital, hospital to home, or hospital to nursing home, can be one of the most vulnerable times for a patient and their loved ones. Pushed from system to system, patients and caregivers can often go through this process alone. Patients are left defenseless by the health care system and often become lost. About the Founder

NY Care Consultants was established by Suzanne Paolucci to help individuals and caregivers receive the best long-term care they deserve. Suzanne is a Licensed Social Worker with over two decades of geriatrics and life care planning experience. She has sought to collaborate with the top Senior Professionals in New York State on issues related to elder and chronic care management. A skilled advocate with a proven track record of ensuring quality of attention along the care continuum, Suzanne is dedicated to helping individuals manage long-term care issues through advocacy, support, counseling, and education.

Facility Advocacy

Individuals receiving attention in a health care facility have the inherent right to be treated with dignity and respect. Patient rights include being well informed about their medical needs as well as being able to actively participate in their own treatment and planning. Often, when these rights are not upheld it leads to patient confusion, frustration and poor quality of care.

Medicaid Advocacy

New York State’s generous Community Medicaid program ensures individuals may remain in the comfort of their home while receiving the care they need. This program includes twenty-four-hour personal care assistance. Through the New York State program, family members could be paid to care for their loved ones. However, recent changes in the law have made accessing this care challenging.

Care Planning

The beginning stages of caregiving can be the most challenging. This is often when the caregiver lacks the necessary knowledge and confidence. NY Care Consultants believes that the caregiver has the most critical role in in the management of their loved one’s illness. They serve as the common thread providing an accurate history of their loved one’s social, medical, and functional needs. Caregivers often play both offense and defense and take the brunt of a confusing and fragmented health care system. They need the skills, tools and above all support to remain confident and well equipped to fill this crucial role.

Care Coaching

Caregiver coaching is designed to tap into the strengths that already exists in a caregiver. NY Care consultants provides caregivers with a personal coach. Individuals will receive one-on-one support to help them understand care options for their loved ones and make informed decisions to meet the challenges and responsibilities that come with being an advocate. The well-being of the caregiver is always at the heart of all care coaching sessions.
